Over the past two years, I have talked to hundreds of health practitioners (some of them not even in Chiropractic, but who have the same problem), as well as done on-site visits and documentation evaluations. 


They were asking me for help and guidance regarding their EHR documentation, or more often, respond to an AUDIT request. They were in PANIC-MODE.

And...they had ALL previously been to MULTIPLE documentation trainings.

There is a REASON why it's so hard to fix your documentation!

It's actually pretty COMPLICATED to FIX existing EHR software notes. There are many interrelated parts to solving the problem of non-compliant documentation, from a thorough understanding of your carrier's guidelines and communicating with patients at ALL staff levels; to office policy and procedures; to your EHR software's capacities, updates, and training; to 'triaging' not only WHAT needs to go into your notes (and why), but also what needs to go AWAY.

It's actually HARDER than starting from scratch with a new program!

COMPLIANT documentation doesn't happen by accident. It is INTENTIONALLY CREATED, and it involves the entire team of an office of facility working together with focused intention. But they need guidance.
